Registered Nurse (RN), Case Manager

Fulltime: Days

Schedule:

Monday-Friday usually 8 hour shifts

Current call matrix is rotating call. Variable weekdays from 3p-7p.

No weekend call

Our Case Management Department primarily serves the adult population in our community. Our hospital has a lower than national average patient ratio. Our team̢۪s approach to offering safe discharge planning is the use of a Utilization RN and Case Manager RN, in conjunction with a Social Worker.

How you will contribute

A Registered Nurse (RN) who excels in this role:

• Accurately performs patient assessments and identifies patient needs

• Identifies and initiates appropriate nursing interventions

• Provides care appropriate to condition and age of the patient

• Performs timely and appropriate documentation relating to medical necessity in the medical record

• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient

• Performs timely and accurate QI assessments

Qualifications:

What we are looking for

Applicants should have a current VA RN license

Bachelors degree preferred but not required

Additional requirements include:

· American Heart Association Basic Life Support

· Experience in Utilization Review preferred

· Requires flexibility in scheduling and ability to work all shift patterns including weekends and holidays to ensure continuity of care and appropriateness of nursing coverage to the patients of Sovah Health Danville on all units.
