About the Role:

The Hospice On Call Nurse Runner (LVN) plays a crucial role in providing compassionate and timely care to patients in hospice settings. This position is responsible for responding to patient needs and ensuring that they receive the appropriate medical attention and support during critical times. The Nurse Runner will collaborate with a multidisciplinary team to assess patient conditions, administer medications, and provide emotional support to both patients and their families. By being available on call, this role ensures that patients' comfort and dignity are prioritized, especially during their final stages of life. Ultimately, the Nurse Runner contributes to enhancing the quality of life for patients and their loved ones through dedicated and responsive care.

Minimum Qualifications:

  • Current Licensed Vocational Nurse (LVN) license in the state of practice.
  • At least one year of nursing experience, preferably in hospice or palliative care settings.
  • Basic Life Support (BLS) certification.

Preferred Qualifications:

  • Experience with electronic health records (EHR) systems.
  • Bilingual skills to communicate effectively with diverse patient populations.

Responsibilities:

  • Respond to on-call requests from patients and families, providing immediate nursing care as needed.
  • Administer medications and treatments as prescribed, ensuring adherence to safety and quality standards.
  • Conduct assessments of patients' conditions and report findings to the hospice care team for ongoing care planning.
  • Provide emotional support and education to patients and their families regarding end-of-life care options.
  • Document all patient interactions and care provided in accordance with hospice policies and regulatory requirements.
  • Available on phone during on-call hours
  • Triage and asses patients condition
  • Hourly Continuous Care when necessary
  • Home visits after hours
  • Can provide quality care for patients and family
  • Communicate with staff members including: Care Coordinators, Registered Nurses, Medical Directors, Intake Coordinators.
  • Completes reports and documentation with quality and in a timely matter.

Effective communication skills are vital for providing clear instructions and emotional support to patients and their families.
